Community Service Forms
- Placement Information Sheet: Students provide this form to their supervisor at the agency where they will perform community service.
- Time Log: The student logs their community service time and has the supervisor verify it on this form.
- Supervisor Checklist: The student’s supervisor fills out this form to rate the performance of the student.
- Student Response: Students answer questions about their community service experience.
Internship Forms
- Parent Acknowledgement Form: Parents sign this form prior to starting an internship.
